2012 Calendar Request Form
Order your 2012 calendars now for yourself or as a gift. Please use this form for all orders, as we will not accept requests by phone.
You can order calendars in two ways:
- Have a salesperson deliver your calendars and they cost you nothing. Limited to certain territories and states.
- Order online and pay a nominal fee. Shipping is free.
Note: We only have a limited quantity of calendars. We reserve the right to limit quantities. Calendars will be available
sometime in December. If you have requested that a salesperson deliver, please allow adequate time for delivery. Payments for calendars will be processed
immediately.
